Back to school is a hectic time of the year for teachers and students! Here’s a fun and free quick survey to get to know your students at the start of a new school year.

Students are asked to list 5, then 4, then 3, then 2, then 1 thing about themselves. Just print and go!

There are two versions included – one has questions already set the other is ready for you to add your own questions. The blank one can be used to review any topic!!

I keep these on hand for new students to fill out and to share as well as an open house activity for students to complete with their parents. I then have students turn it into a paragraph and I post it in the hall by the classroom! I also can incorporate it into many different themes such as super heroes, Hollywood "Walk of Fame," and so on.

I appreciate a beginning of the year resource with the focus on the student, as well as the timeliness of the topic! With the movie "The Fantastic Four" out, the students would enjoy the implied reference.

Fantastic resource for getting to know students at the beginning of the year and gaining a basic idea of what they remember from last year. I will definitely be using with my 7th graders!
